Once you have meet all certification requirements and passed the exam, you will need to complete the application process using the form below.

You will need to complete the application in one sitting. You cannot save a partially completed application, so be sure you have all of your documentation together before you begin.

If your application is missing CE certificates, information, and or exam score, the system will not let a 'Reviewer" see the incomplete application. Application fees are non-refundable, so be sure your application is accurate prior to submission.
